Hi there Dominic, thanks for posting the pictures.
It looks like the rust has been building up for some time from the inside (water trap?) and has worked its way through.
I would think the only way to repair this with any degree of permanence would be to get the offending section cut away, and a new bit welded in.
Filler wouldn't last more than a few months (if you are lucky), before the rust started poking through again, and you would incur the same cost/hassle of painting, whichever method you choose.
It may be time to look at the overall cost of the repair versus the condition/value of the car and to decide before commiting funds whether the car is 'worth' saving.
