There's nothing said about what the car failed on, but I wonder about the wheel nuts not being replaced correctly. I am not aware of the removing of road wheels being part of the mot.
They're not. But if an MOT inspector checked for play in the wheels, joints etc (as they must) it could have been evident.
From the story I reckon another failure was with the seat belt.
