Regarding what could be perseved as causing the issue, well Andy-Tech gave us a very good, concise technical and operational summary:
This isn't saying it IS maintenance that is causing, but just advistory steps to prevent it happening. Problem is the part is designed to "stick" -- both the rod itself (spring part of the mechanism) and the catch (bonnet hole) -- so the part is not well-designed in my opinion.
If you compare this to the new mechanism (albeit linked to a different bonnet design), the mechanism seems one that even with little maintenance won't "stick".
The current design is inferiour and feeble - from the main catch right through to the safety catch!
