If its getting worse at hot idle, Perhaps you have a split or leaking breather hose somewhere?
If the MPG is worse following O2 sensor replacement, it tells you the spark or the fuel are not right.
(Provided the air mix is correct)
Any warning lights on? Knock sensor should pick up ruff running.
Have you had the timing checked?
