Has anyone had this happen to them?

I was wondering if anyone else has had this happen to them?

It seems the friction material has seperated from the plate there is a good 6mm left on the pad however I now need to replace both front discs and pads

the brakes were cold and the car was driven 15m when I tried the brakes my foot went to the floor I was only doing 5-10 mph as aproaching a junction pumped brake pedal the pedal returned to normal pressure car turned round and parked it up this is what I found on removing the wheel
