Whack whats left of the screws with a sharp punch.They normally go.If not you've lost nothing and you will then have to drill off the (just) enough of the heads to get the disc off.When the disc is off there should be plenty to grab hold of with mole grips.
Id make sure the caliper is moving as it should and the piston/s arnt seizing,especially on the side where the pads worn oddly.

Changed brake disc on drivers side and pads, grinding has gone, nearly, can still slightly feel it, hope this goes when i change other side which hasnt been done yet cos i got sent the wrong disc
