Do code readers ever get it wrong?

My Megane scenic (currently in limp mode) has been diagnosed (via a code reader) with faulty fuel injection circuitry. Another mechanic has said that particular fault would not have put the car into limp mode and suggests that the gearbox needs to be stripped down and rebuilt. In your opinion(s), are the ECU error codes known to get it wrong sometimes?
