A grinding noise from the brakes is one sign of metal touching metal. A very unsafe position to be in as any heat generated by the worn pad will be immediately transferred to the piston caliper with the risk of the brake fluid over-heating or caliper damage - either can be an expensive problem.
On the other hand it just could be dirt or grime but you'll not know until you have made a full inspection.
