I normally clamp the flexi-hose that way there is little risk of air getting back up the brake lines towards the mater cylinder although it is good practice to remove the master cylinder cap first just in case.
Once everything is assembled I open the bleeder unclamp the hose and let gravity do the rest. Once fluid is running clear a few pumps of the pedal for a final bleed is usually sufficient.
