Success, but not as I expected.
I removed the speaker as per Steve's tip. There's not much room to get your hand in and I couldn't see the mechanism. After feeling around I could tell the sprung roller mechanism was loose. I then disconnected the stay from the door hinge pillar and opened the door as far as possible. I could then see that one nut holding the mechanism to the door was missing and the other half undone.
I replaced the missing nut and tightened the other. All now as good as new, if not better if it came off the track with loose nuts that got looser with the years. It's not good having loose nuts!!
Checked the other three doors and all were OK.
As a footnote, when refitting the speaker I dropped one of the screws on the floor and it rolled under the car. I got down to recover it and next to it was the missing nut. How on earth that got there I don't know.
Check your nuts lads.
