Unusual brake pad wear

I took off the clios brake pads today just to give the brakes a quick clean, but noticed this:

The tops of the pads have not been used at all, the disk is cutting a groove into the pads. I changed these pads and disks about a year or two ago, could too small a disk have been fitted?

The brakes work but lack the stopping power of my 406 and i have found the abs likes to kick the pedal back at really slow speeds
